HomeCare of York’s Mission
The mission of HomeCare of York is to provide excellent home health services in a personalized, cost-effective manner by anticipating and meeting the changing needs of the community we serve.
What is HomeCare of York?
HomeCare of York is a Medicare/Medicaid certified, state-licensed home health agency dedicated to providing medical care and social services to patients in their home environment.
HomeCare of York nurses appreciate the privilege of caring for patients in their home. Our individualized programs are designed to keep a patient where they want to be – at home. We recognize the innate worth of all individuals and the belief that each individual is entitled to maximize his/her potential as a human being and as a member of society.
To be eligible for services, patients must be homebound, require medically necessary care, require care on a part-time or intermittent basis and have physician orders for care. After consulting with the patient’s physician, a primary care nurse is assigned to ensure the patient’s needs are met. This registered nurse coordinates a team that administers a treatment program to best suit each patient, incorporating the necessary physical, occupational and speech therapy, medical social services and more. Our staff will also help the patient obtain home care equipment and supplies needed during the course of care with us.
The HomeCare of York team teaches patients and their families how to perform tasks and to help maintain and improve the patient’s health. A professional nurse is available 24-hours a day to answer questions and provide care.
