Memorial Hospital’s Diabetes Self-Management Education Program is committed to promoting, improving and enhancing the health and well-being of the diabetic community. If you were recently diagnosed with diabetes or even if you have had it for a long time, you probably have many questions. Memorial’s Diabetes Self-Management Education Program can guide you through your questions and provide you with a solid foundation to effectively manage your diabetes.
The American Diabetes Association recognizes Memorial’s Diabetes Self-Management Education Program as meeting the national standards for diabetes self-management education.
PROGRAM FORMAT
The Diabetes Self-Management Program consists of a one-hour pre-assessment interview and three 3-hour classes. The program offers information about managing and monitoring diabetes and focuses on general information about diabetes and the aspects of self-care. The classes utilize a comprehensive, interdisciplinary team approach to meet the National Standards for Diabetes Self-Management Education.
PROGRAM TOPICS
The topics covered during the Diabetes Self-Management Program include:
General explanation of diabetes
Carbohydrate counting
Medications
Monitoring and blood glucose control
Fiber, hidden sugars and alternative sweeteners
Reading food labels
Exercise
Preventing, detecting and treating acute complications
Risk reduction
Restaurant eating and recipes
Coping with diabetes
Post-test, course evaluation and presentation of certificates
PROGRAM FEE
The cost of this program is reimbursable by many health insurance companies, including Medicare. Consult your insurance carrier for information about your coverage. Please call Memorial’s Education Resources Department at 717-849-5461 for class fees.
